If this is the case, please print off and use the documents below to screen and complete the process electronically at a time convenient to you.  Completing the tool electronically records the fact that a screen has been undertaken, scores the screen for you, and gives you information to copy into your electronic notes at the end of the process. 

Any screens which are carried out on paper, and not then copied into the online version are not ‘countable’.
